The Rail Accident Investigation Branch (RAIB) has confirmed that it is investigating a fatal accident at Ickenham station on the London Underground. At approximately 22:30 on 28 March 2025 a passenger fell from the eastbound platform onto the track and was later struck by a train. London Underground staff found the injured passenger, who sadly succumbed to injuries at the scene.
According to the preliminary notice published by RAIB on 16 April 2025, the investigation will focus on four principal areas:
- Sequence of events – establishing the precise timeline from the passenger’s arrival on the platform to the impact with the train.
- Human factors – analysing the actions of all parties involved and any influences on those actions, such as fatigue or distraction.
- Staff management and competence – reviewing training records, supervision practices, and competence assessments for the station team.
- Risk management arrangements – evaluating existing measures designed to prevent track incursions and to detect individuals on the line.
RAIB emphasises that its inquiry is independent of the operator’s internal review and of any enforcement action by the Office of Rail and Road. Findings and safety recommendations will be published in a final report on the RAIB website once the work is complete.
Ickenham station, situated in the London Borough of Hillingdon, is served by both the Metropolitan and Piccadilly lines and has two platforms. RAIB’s outcomes could therefore influence future risk-mitigation measures not only at Ickenham but across comparable surface stations.
